The test room's output, recorded properly. Thresholds go in as a grid — one row per line, one column per frequency — because that is the shape of the form the audiologist already has on paper.
The audiogram is then drawn from those numbers, not scanned or uploaded. That matters: it prints sharply at any size, it is readable on a phone, and two tests can be put side by side to show whether hearing has changed. Right is a red circle, left a blue cross, bone conduction dashed — the convention every audiologist already reads.
The pure-tone average is computed over the frequencies that were actually tested, so a test that skipped 4 kHz is not averaged against a zero and made to look better than it is. Degree and type of loss follow from the thresholds and the air-bone gap, and the report says so unless the audiologist writes their own impression.
Which frequencies you test, and which go into the average, are settings — a centre that adds 3 and 6 kHz does not need a new version of the script.
