Audiology & Clinical Records

On this page

The Audiology & Clinical Records addon keeps what the test room produces on the patient's file: pure-tone thresholds ear by ear, speech results, impedance, and the audiologist's reading of them. You type the numbers; the audiogram is drawn from them. It never goes blurry on a phone, and the next test can be laid beside it.

It is for the audiologist and the doctor, and for the front desk who has to find last year's result when the patient walks in again.

Switching it on

Audiology & Clinical Records is a premium addon, so it must be in your licence.

  1. Buy it from its card on Admin → Addons (Buy this addon), or on the License Center at https://license.faruk.bd. A paid order arrives by itself at the next licence check.
  2. If you have the key already, open License, paste it under Addon keys and press Add the key. Check now asks the License Center at once.
  3. Press Enable on the Audiology & Clinical Records Addons card.

Its screens sit under Patients & Clinic, beside the patient list: Hearing Tests and New Test. The settings open from Admin → Addons → Settings on the card.

The dashboard shows Hearing tests recorded in the last 30 days.

Permissions

Three permissions come with the addon, given to roles in Users & Roles:

PermissionWhat it allows

Settings

SettingWhat it does
Frequencies tested (Hz)Default 250,500,1000,2000,4000,8000. Comma separated. 3000 and 6000 are worth adding if your audiometer has them.
Frequencies averaged for the PTADefault 500,1000,2000,4000. The four-frequency average is the usual one in Bangladesh; 500,1000,2000 is the classic three.

Recording a test

New Test, or Record a test on the patient's Hearing tests tab.

  1. Patient: choose the patient the test belongs to. From the patient file it is filled in.
  2. Test date, Test (Pure tone audiometry, Speech audiometry, Impedance / tympanometry, OAE, BERA / ABR or Free field), Tested by, and Branch when you run more than one (Multi-Branch).
  3. Thresholds (dB HL): one row each for Air conduction — right, Air conduction — left, Bone conduction — right and Bone conduction — left, one box per frequency, in steps of 5 dB. Leave a box empty where nothing was tested; an empty box is not zero. Tick Masked (right) or Masked (left) when masking was used.
  4. Speech & impedance: SRT right (dB), SRT left (dB), Discrimination right (%), Discrimination left (%), Tympanogram right and left (A, As, Ad, B or C), and Reflexes right and left (e.g. Present / absent).
  5. History & reading: Presenting complaint, Relevant history, Otoscopy, Impression, Recommendation, Remarks, and Attach a scan or photo (an image or PDF).
  6. Press Record the test (Save the test when editing). The panel says Test recorded as AUD-2026-00012.

Leave Impression blank and the report prints the degree and type of loss worked out from the thresholds.

What the addon works out

For each ear, from the thresholds:

  • PTA: the average of the frequencies you chose.
  • Degree, from the PTA:
PTA (dB HL)Degree
up to 25Normal
over 25, up to 40Mild
over 40, up to 55Moderate
over 55, up to 70Moderately severe
over 70, up to 90Severe
above 90Profound
  • Type: Normal, Conductive, Sensorineural or Mixed. An average air-bone gap of 15 dB or more means conductive, or mixed when the bone conduction itself is above 25 dB; a smaller gap means sensorineural. It needs both air and bone results.

On the audiogram, the right ear is red and the left ear blue: air conduction as O (right) and X (left) joined by a line, bone conduction as < and > joined by a dashed line.

Hearing Tests

The list shows every test with its Date, Patient, Test, the Right and Left result (PTA and degree, coloured from mild to profound) and who Tested by. Search by patient name, number or reference, filter by test type, and press Filter. + Record a test starts a new one.

The report

Opening a test shows the Audiological Report: the patient, the drawn audiogram, a box for the Right ear and the Left ear (PTA, degree, type, SRT, discrimination, tympanogram, reflexes, masking), the history and reading, and your footer line. Print gives a clean page for the patient's folder. Edit and Delete are beside it.

Below, Earlier tests for this patient lists their other tests with the right and left PTA, so a change over the years is easy to see.

On the patient file

The patient's file has a Hearing tests tab: date, type, right and left PTA, the reading, and Open for each test (see Patients).

What the patient sees

A patient with an account on your website (with the Patient & Customer Management addon) sees Your hearing tests: the dates and the kind of test, nothing more. The numbers and the diagnosis stay with you, because a patient reading "moderate sensorineural" alone at eleven at night, with nobody to ask what it means, helps nobody. The page tells them to ask you for a copy. To give a patient their report, publish it with the Online Lab Reports addon.

A typical visit

  1. The patient arrives for an appointment and is marked Arrived (see Appointments).
  2. The audiologist opens the patient file and presses Record a test.
  3. The thresholds go in as the test is done; the audiogram is ready the moment it is saved.
  4. The doctor reads the report, writes the Impression and Recommendation, and prints it.
  5. If a hearing aid is advised, the trial and fitting are recorded with Hearing Aid Management.

Tips

  • Set your frequencies before the first test. Adding a frequency later is fine; old tests simply have an empty box there.
  • Never type 0 for "not tested". Leave the box empty.
  • For fully designed printed reports (audiogram, impedance, SRT and endoscopies, each with your own header and footer), see Online Test Report Creator.
