Online Test Report Creator

On this page

The Online Test Report Creator addon writes and prints every test report a hearing and ENT centre produces, each laid out the way you want it on paper:

ReportNumber starts
PTA Hearing Test (Audiogram)R-PTA
Impedance / TympanometryR-IMP
SRT ReportR-SRT
Ear Endoscopy (Otoendoscopy)R-OTO
Nasal Endoscopy (Nasoendoscopy)R-NAS
Fiber Optic Laryngoscopy (FOL)R-FOL

Each kind has its own print design and a phrase bank that offers back what you have written before. It is for audiologists and ENT surgeons who print reports every day.

Switching it on

Online Test Report Creator is a premium addon, so it has to be in your licence first.

  1. Buy it with Buy this addon on its card on Admin → Addons. When the order is confirmed, the key comes down by itself at the next licence check; Collect purchases now under License → Addon keys fetches it at once. The addon is normally switched on as it arrives.
  2. A key you got some other way goes in the Addon key box under License → Addon keys. Paste it and press Add the key.
  3. If its card on Admin → Addons still says Disabled, press Enable.

Where to find it

Under Patients & Clinic in the sidebar: Test reports (write, find, print), Print design and Phrase bank. Its settings open from Admin → Addons → Settings.

The permissions, in the Test reports group under Roles & Permissions, are Read and print test reports, Write, edit and delete test reports and Change the print designs and the phrase bank.

Writing a report

On Test reports, the card Write a report shows the six kinds; pick one and it opens ready to fill in. The Test reports tab of a patient's file has the same six buttons with the patient already chosen.

The patient. Pick a Patient on file and the boxes fill in, or choose Not on file — type the details below for somebody referred in only for a scope. Name is required; Referred by and Report date are printed too.

The audiogram. Type each threshold into the table (right and left, air and bone, 250 Hz to 8 kHz), or click a chart to place a point; hold Alt while clicking for bone conduction. A blank box means not tested and is left out of the average. Right ear — note and Left ear — note take remarks such as No response at 100 dB, masked. Each chart shows the four-frequency average, its grade (within normal limits to profound hearing loss) and the hearing handicap as a percentage.

Impedance and SRT. Under Readings, both ears side by side, the way the machine prints them: tympanogram type, volumes, pressures and reflexes, or SRT, MCL, UCL and discrimination score. Leave anything not tested empty.

The endoscopies. Under Pictures, Add a picture fills the five usual slots and + Add more pictures adds more. Each picture has a Caption (printed under it), a Printed width and Remove. Take the dark surround off automatically removes the black circle around a scope image, in your browser.

Findings. Every kind ends with headed boxes: Comments or Impression and Recommendation for the hearing tests; RIGHT EAR, LEFT EAR, DX and Notes for the ear endoscopy; anatomical headings for the nasal endoscopy and FOL.

  • Start typing and lines you wrote before come back as suggestions.
  • Suggest from the numbers writes a first draft of the comment from the readings. Read it and change it.
  • Untick Print on a box to keep it off the paper without losing it. A normal finding is worth recording, not always worth printing.

Signature. Signed by and Position start with the standing name from the print design; whoever wrote the report can put their own.

Saving. Save keeps the report as it is. Save & print finishes it and opens it ready to print. Back to draft takes a finished report off the patient's own page.

Test reports lists every report, searchable by name, report number or phone, with Print, Edit and delete. The dashboard shows Reports today.

Print design has a tab for each kind. Until you change anything, a design uses the centre's own name, address, phone, e-mail and the logo from Theme, so reports print properly the first time.

SectionWhat you can set
The headerPrint it; a Logo and its height; the Arrangement; Centre name, Telephone, E-mail, Website and Address, each with Print this line; Space above the header (px) for pre-printed paper.
The pageReport title, Print the title, Paper (A4, Letter), Text size (px), Page margin (mm), Print the patient block, Print who referred them.
Signature & sealName, Position, Signature / seal picture (a transparent PNG sits best), Line under the signature, such as your BMDC registration number.
The footerFour optional lines and a Footer picture: a branch list, an accreditation, partner logos.

Press Save this design. Copy a design copies another kind's design over this one; Start again goes back to the centre's own details.

Phrase bank

Phrase bank has a tab for each kind. Under Write some in, choose Which box, paste your standard findings into The phrases, one per line, and press Add them. What is in it lists every phrase with how often it was used; Forget removes one and Empty it clears a box or a whole kind.

What the patient sees

Finished reports appear under Your test reports in the patient's account on your website, each with Open. A report opens only for the patient it was written for, signed in to their own account; a forwarded link shows Report not available. Drafts are never shown.
